1. Typed and signed application with passport photo affixed.
2. Highest Academic Certificate.
3. Local Government Area of Origin Certificate.
4. Birth Certificate/Age declaration.
5. NYSC Discharge/Exemption Certificate, where applicable.
6. All certificates must be from Government recognized institutions and uploaded from originals.
